how often are pest/ vermin inspections or eradications carried out in your service and how are these records maintained? How often should these inspections be carried out?

Not sure if this is right, but i put this down.
1) Who applied the Inspection or eradication
2) When the inspection and/or eradication was carried out
3)Details of the pesticide/chemicals that were used such as ingredients and quantity
4)Details of where the location of pesticide/chemical/inspection was used or done.
