Create an A4 report pro forma you could use for each child: 1 sheet per child, per book. This report will be submitted to Mrs. Mack at the conclusion of each book study and should review each child’s progress, and notes about the children’s outcomes with each activity. You only need to place headings on the page (as you do not have the information to fill in the form). Be sure all relevant information is on the format, and that the headings you use provide Mrs. Mack with information about the child’s progress and skills for each activity.
