Barriers to learning for children with additional needs?

Hi can someone help me with the following question?

Describe practices that are in place to identify and address any barriers to learning for a child with additional needs. (Think about accommodating physical needs, additional equipment or additional staffing/support services)

- is conducting an Inclusion Support Plan right?
