I've got a sample of a recruitment report which you can see and then possibly use to create your own sample. This is from a different company but you can get an idea on what you need to include in it etc. I am also providing you with a copy of "Recruitment & Selection Guide" for children's services. You may be able to use some of the information from here in the recruitment report.

Thank you lot of that helps with the 1st part of this Assessment, but not to sure about the Centre Report but thank you so much for your quick reply u are great .....

I'm not sure if the consultative evaluation process is referring to the centre report from question 1 or it's a separate question altogether. However if it is a separate question I think consulting should include staff, families, children, stakeholders etc and evaluating processes can be done in the form of feedback forms, questionnaires, input, surveys, open questions etc based on centres policies, environment etc.
