hi, I need some help to see if I'm on the right path.
Question is - develop a support and inclusion plan for a child.

I also would have observations with this to help and a milestone checklist to help support this and help for outcomes goals to work on. have communication book with parents on discussions and progress, also meeting notes. if the child had a disability for example in a wheelchair is the service wheelchair approved ? is there changes we need to make it more appropriate for this.

do I need to fill this plain out based on a child with a special needs ?

A child with additional needs doesn't necessary have to have a physical disability, it could also be a child with a behavioural issue, a child from a different culture...
